California-based Kaiser Permanente Health Plans is the largest payer in the U.S., covering about 9,257,115 patients and incurring $48.19 billion in claims, according to data published Feb. 20 by Definitive Healthcare

The 20 largest payers by incurred claims: 

1. Kaiser Permanente Health Plans (Calif.): $48.19 billion

2. UnitedHealthcare (Minn.): $31.20 billion

3. Elevenace Health (Ind.): $30.99 billion

4. Health Care Service Corporation Group (Ill.): $27.09 billion 

5. Guidewell Mutual Holding Group (Fla.): $13.89 billion

6. Centene Corporation (Mo.): $13.42 billion 

7. Blue Shield of California: $12.20 billion

8. Aetna (Conn.): $11.88 billion

9. Cigna (Conn.): $11.63 billion

10. CareFirst BlueCross BlueShield (Md.): $7.20 billion

11. BlueCross BlueShield of Michigan Group: $6.40 billion

12. Highmark (Pa.): $6.06 billion 

13. BlueCross BlueShield of NC Group: $6.03 billion

14. BlueCross BlueShield of Massachusetts Group: $5.22 billion

15. Horizon BlueCross BlueShield of New Jersey: $5.10 billion
