Under the 2014 Medicare Physician Fee Schedule final rule, guidelines for chronic care management payments have been changed, and the changes will start in 2014, according to AAPC.
In 2015, the fee schedule will pay separately for chronic care management services when patients have multiple conditions that last at least one year and that place the patient at significant death risk.
