- Edits: 44 percent
- CCI issues (inappropriate coding): 24 percent
- Unbundling (billing for multiple procedures covered by one comprehensive code): 19 percent
- Medical: 7 percent
- Other: 4 percent
- Medically unlikely edits errors: 2 percent
