12 California Hospitals Fined for Serious Patient Safety Errors

The California Department of Public Health has announced 12 California hospitals have been assessed fines totaling $650,000 after it was determined the facilities' noncompliance with licensing requirements caused, or was likely to cause, serious injury or death to patients.

 

The following hospitals received penalties (hospitals are listed in alphabetical order by name):

1. Alameda Hospital: The hospital failed to develop and implement written policies and procedures to ensure the safe and effective administration of medication. The penalty is $50,000.

2. Brotman Medical Center in Culver City: The hospital failed to ensure the health and safety of a patient when it failed to follow its policies and procedures for fall prevention. The penalty is $50,000.

3. California Men's Colony in San Luis Obispo: The hospital failed to implement written policies and procedures to ensure the safe and effective administration of medication. The penalty is $50,000.

4. Dominican Hospital in Santa Cruz: The hospital failed to develop and implement written policies and procedures to ensure the safe and effective administration of medication. The penalty is $75,000.

5. Emanuel Medical Center in Turlock: The hospital failed to ensure the health and safety of a patient when it did not follow its surgical policies and procedures. The penalty is $75,000.

6. Kaiser Foundation Hospital & Rehabilitation Center – Vallejo: The hospital failed to ensure the health and safety of a patient when it did not follow its surgical policies and procedures. The penalty is $50,000.

7. LAC+USC Medical Center in Los Angeles: The hospital failed to ensure the health and safety of a patient when it failed to implement written policies and procedures to ensure the safe and effective administration of medication. The penalty is $50,000.

8. Riverside Community Hospital: The hospital failed to ensure the health and safety of a patient when it did not follow its surgical policies and procedures. The penalty is $50,000.

9. Stanislaus Surgical Hospital in Modesto: The hospital failed to ensure the health and safety of a patient when it did not follow its surgical policies and procedures. The penalty is $50,000.

10. Sutter Delta Medical Center in Antioch: The hospital failed to ensure the health and safety of its patient when it failed to follow its policies and procedures related to ongoing patient monitoring and assessment and provision of patient care. The penalty is $50,000.

11. Torrance Memorial Medical Center: The hospital failed to ensure the health and safety of a patient when it did not follow its surgical policies and procedures. The penalty is $50,000.

12. UCSF Medical Center, San Francisco: The hospital failed to ensure the health and safety of a patient when it did not follow its surgical policies and procedures. The penalty is $50,000.

 

When hospitals receive their survey findings, they are required to provide CDPH with a plan of correction to prevent future incidents. Hospitals can appeal an administrative penalty by requesting a hearing within 10 calendar days of notification.
