Bedford (N.H.) Ambulatory Surgical Center has just completed a 15-month pilot program through Harvard Pilgrim Healthcare doing concept testing for patient-centered medical homes, according to a report from the New Hampshire Union.
The ASC was one of several collaborators for the pilot, including Foundation Medical Partners, the physicians group of Southern New Hampshire Health Systems and Harvard Pilgrim Health Care, which organized the effort.
During the program, the organizations experimented with expanded hours and access to care, broad use of electronic medical records and walk-in services. The efforts decreased emergency department visits by approximately 12 percent and boosted immunization rates, according to the report.
