The questionnaire is organized by seven criteria categories — leadership; strategic planning; customer focus; measurement, analysis and knowledge management; workforce focus; operating focus; and results. These categories align with the Baldrige Criteria for Performance Excellence, which are requirements for receiving the Malcolm Baldrige National Quality Award.
