Method to prevent pediatric medical errors: 4 things to know

Authors of a new study on reducing pediatric harm developed a Pediatric All-Cause Harm Management Tool to detect if "an adverse event may have occurred," during medical practice according to a Med Page Today report.

Here are four things to know:

1. David C. Stockwell, MD, MBA, of Children's National Health System in Washington, D.C., and colleagues found 45 percent of harms assessed from 600 reviewed charts were "potentially or definitely preventable," according to the report.

2. PACHMT identified 85 percent of all harms, as well as 70.6 percent of all triggers at least once.

3. PACHMT was modeled from the Institute of Healthcare Improvement Global Trigger Tool.

4. According to the study abstract, the PACHMT uses a modified Delphi method, and researchers note, "Pilot testing this list of pediatric triggers to identify all-cause harm for pediatric inpatients is the next step to establish the appropriateness of each trigger for inclusion in a global pediatric safety measurement tool."
