Andrews Institute Ambulatory Surgery Center (Gulf Breeze, Fla.). The Andrews Institute Ambulatory Surgery Center was founded as a joint venture between physicians and Baptist Health Care, and includes an extensive physical therapy program. It is affiliated with Andrews Institute for Orthopaedics & Sports Medicine, founded by James Andrews, MD, who is renowned for his work with professional athletes and serves as team physician for the Tampa Bay Rays. The rehabilitation program includes physical therapy, occupational therapy and aquatic therapy. The programs are focused on treating several patients, including sports injuries.

Beacon Orthopaedics and Sports Medicine Ambulatory Surgery Centers (Cincinnati and Sharonville, Ohio). Beacon Orthopaedics and Sports Medicine includes an ambulatory surgery center and physical therapy services at both its Summit Woods and Beacon West locations. Surgeons are able to perform several types of procedures at the facilities, which include 23 hour stay nursing care rooms. The surgery centers are accredited by the Accreditation Association for Ambulatory Health Care and have viewing rooms where families can see into the OR while a procedure is being performed. The physical therapy space includes more than 5,000-square-feet of work space.

Cascade Orthopaedic Ambulatory Surgery Center (Auburn, Wash.).
Cascade Orthopaedic Ambulatory Surgery Center was established in 1999 and moved to a new facility in October 2000. Founded by the surgeons of Cascade Orthopaedics, the surgery center includes three operating rooms and fully equipped preoperative and postoperative anesthesia care units. Surgeons perform several orthopedic procedures, including arthroscopic surgery, at the center on a daily basis. Cascade Orthopaedics also includes a spine center, diagnostic imaging, physical therapy, massage therapy and ultrasound services. The center is Medicare-certified and accredited by the Accreditation Association for Ambulatory Health Care.

Central Main Orthopaedics (Auburn).
Central Main Orthopaedics is a 42,000-square-foot orthopedic ambulatory surgery center that includes a physical therapy center. In 2010, Central Maine Orthopaedics created Maine Spinecare to foster collaboration between operative and non-operative spine specialists. The practice has an internal exercise gym with spine therapists who have advanced training in back and neck pain. The practice also includes X-ray, MRI diagnostic and an injection suite. The ASC has two operating suites and is accredited by the Accreditation Association for Ambulatory Health Care. The surgeons perform around 2,000 surgeries per year, including arthroscopic surgery, extremities care and ACL procedures.

The Center for Orthopedic Surgery (Van Nuys, Calif.). The Center for Orthopedic Surgery was developed and operated by physicians at Southern California Orthopedic Institute and is located within the practice. There are 32 physicians at the institute, performing a variety of orthopedic surgeries. The center pioneered the development of arthroscopic surgical procedures, particularly in the shoulder, wrist and ankle. Additional services include physical therapy and MRI. Physicians at the center are involved with the Southern California Orthopedic Research and Education Center, a non-profit established in 1998 for research, education and publishing ventures. The Center for Orthopedic Surgery is accredited by the Accreditation Association of Ambulatory Health Care.

Edmonds (Wash.) Orthopedic Outpatient Surgery Center.
The outpatient surgery center of Edmonds Orthopedic Center is accredited by the Accreditation Association for Ambulatory Health Care. Physicians affiliated with the center have a range of special interests, including minimally invasive knee surgery, sports medicine and shoulder care. The ASC includes three operating rooms, one diagnostic room and a dedicated radiographic C-arm. Edmonds Orthopedic Center also includes physical therapy and hand therapy services as well as MRI.

Evergreen Orthopedic Clinic Surgery Center (Kirkland, Wash.).
Evergreen Orthopedic Surgery Center includes physicians who subspecialize in sports medicine, spine and extremities care. The surgery center is on the first floor of the facility, with the second floor housing Evergreen Orthopedic Clinic and Evergreen Orthopedic Physical Therapy. Evergreen Physical Therapy offers sports rehabilitation, gait analysis and orthotics, postural and biomechanical training and hand therapy. MRI diagnostics is also available at the surgery center, which is accredited by the American Association of Ambulatory Health Care.

Minimally Invasive Spine Institute Health Campus (Dallas).
This 48,000-square-foot center opened in April 2011. It features four ORs, one minor procedure room and 16 recovery areas — all of which are private rooms. As part of a phased opening, it started with self-pay patients and then began accepting private insurance patients. The facility takes a holistic approach to healthcare by providing a full spectrum of treatment options, from physical therapy to surgical intervention. It is among the first in Texas to host endoscopic laser spine surgery.

OA Centers for Orthopaedics (Portland, Maine)
. The OA Physical Therapy Center is located down the hallway from OA Surgery Center, which allows the physical therapists to schedule appointments promptly after surgery. Physical therapy services include re-habilitation for athletes, acute injury therapy, postoperative recovery, transitional sports rehabilitation and sports specific biomechanics assessment. The surgery center is accredited by the Accreditation Association for Ambulatory Health Care. The expanded 10,000-square-foot facility includes two digital operating rooms, onsite X-ray, MRI and new procedure rooms. It was opened in 2003.

Optimorthopedics Ambulatory Surgery Center (Savannah, Ga.).
Optimorthopedics includes a full-service outpatient surgery center, imaging center, physical rehabilitation center and a hand and upper extremity clinic. The physical therapy services are provided by D1 and focuses on training several types of patients, including the elite athlete. The ambulatory surgery center is accredited by the Accreditation Association for Ambulatory Health Care and provides expertise in sports medicine, joint replacement, spine care, trauma, extremities and pain management.

Orthopedic Surgery Center of San Antonio (Texas).
The Orthopaedic Surgery Center of San Antonio is the surgery center of The San Antonio Orthopaedic Group, which was established in 1947. Ancillary services at the ASC are comprised of MRI and CT scans as well as physical and hand therapy. The group also offers industrial rehabilitation, work hardening and work conditioning. The physicians have a professional interest in arthroscopic surgery, sports medicine and extremities care.

Pacific Surgical Center (Longview, Wash.).
Pacific Surgical Institute houses Pacific Surgical Center as well as Longview Physical & Sports Therapy. The therapists offer comprehensive orthopedic and manual therapy, sports and recreation treatment and industrial injury treatment, among other services. The 40,000-square-foot Institute also houses Longview Urology, Pacific Imaging Center, Longview Orthopedic Associates, Northwest Medical Analytic Laboratory and Flourish Skin & Laser. The $10 million project is backed by seven physicians and was opened in June 2006.

Prairie SurgiCare (Peoria, Ill.).
Prairie SurgiCare has 23-hour accreditation from the Accreditation Association for Ambulatory Health Care and was added to Prairie Spine & Pain Institute in April of 2012. Richard Kube, MD, performs several spine-focused procedures at the surgery center, including single- and multilevel artificial disc replacement. Throughout this past year, clinical services have expanded at the practice to include quality of life care and rehabilitation programs. The 5,700-square-foot facility includes physical therapy, with both manual and message therapy.

Presidio Surgery Center (San Francisco).
Presidio Surgery Center was initially founded in 1989 as a joint venture with Sutter Health. It is now a joint venture between several physician groups and Sutter. The facility includes five operating rooms and one procedure room where physicians perform approximately 6,000 cases annually. There are several types of procedures performed at the center, including hip arthroscopy, minimally invasive spine surgery and pain management. The surgeons are also beginning to perform minimally invasive joint replacements. There are 41 physician partners and the orthopedic surgeons provide care for the Oakland Athletics, San Francisco Ballet and Golden State Warriors. In addition to the surgical services, the center includes physical therapy for joint replacement patients. Further specialties at the surgery center include urology, ENT and general surgery.

Southeastern Spine Institute Ambulatory Surgery Center (Mount Pleasant, S.C.).
Southeastern Spine Institute Ambulatory Surgery Center is South Carolina's only ASC dedicated specifically to spine health. The facility was designed by Don Johnson, MD, and Steve Poletti, MD, to address patients with spine-specific issues, including total disc replacement. In addition to outpatient surgery, Southeast Spine Institute includes imaging, fluoroscopy and physical therapy. The surgery center is accredited by the Accreditation Association for Ambulatory Health Care and Medicare-certified.

Tallahassee Outpatient Surgical Center.
Tallahassee Outpatient Surgical Center is a partnership between Tallahassee Orthopedic Clinic physicians and Hospital Corporation of America. The clinic also includes Tallahassee Orthopedic and Sports Physical Therapy, which is an independent physical therapy center working within the orthopedic center. Additional units include the MRI division and the division of orthotics and prosthetics. The physicians care for patients with sports injuries, orthopedic trauma and extremities conditions. TOC was founded in 1972 as the first practice in the area to focus on orthopedics.

Upstate Bone & Joint Center (Syracuse, N.Y.).
The physicians of Upstate Orthopedics perform orthopedic and pain management procedures at Upstate Bone & Joint Center's ambulatory surgery center. Their 100,000 square-foot facility includes the ASC, imaging services, custom orthotics and prosthetics, physical therapy, occupational therapy and a lab for blood work. There are 16 orthopedic surgeons who have a special interest in sports medicine, extremities care and spine.

Virginia Spine Institute (Reston). The Virginia Spine Institute is a medical practice in the Washington, DC metro area solely dedicated to comprehensive spinal healthcare. They have assembled physicians from multiple specialties of spinal healthcare under one roof to provide convenience of specialty care to improve the lives of those suffering from neck or back pain. These services include non operative care, pain management, physical therapy, and surgical intervention. The physical therapy sector of the Virginia Spine Institute, Virginia Therapy & Fitness Center, promotes the general wellness and health needs by providing evidence-based physical therapy, athletic training, sports performance training, nutritional guidance and fitness.
