1. Write queries clearly, concisely and precisely.
2. Include case evidence.
3. Do not lead the query.
4. Be part of the clinical documentation.
5. Introduce ICD-10 language.
Coders should also note that an incorrect ICD-9 diagnosis will still be incorrect when translated to ICD-10. Maintain accuracy now for better results with the updated code set.
