Taking on progressive payment models: 3 ASCs offering bundled payment options

Here are three surgery centers that offer bundled payment options for certain procedures.

Bedford (N.H.) Ambulatory Surgical Center. The ASC has offered bundled payments for colonoscopy and esophagogastroduodenoscopy to uninsured patients for several years, according to a Becker's ASC Review article. In May, it announced that it had initiated a bundled payments pilot program with health plan Harvard-Pilgrim Healthcare. The program includes routine colonoscopy, and the bundled rate includes fees for the surgeon, anesthesiologist and all other clinicians.

The surgery center was established in 1993 and is accredited by the Accreditation Association for Ambulatory Health Care. It has performed 124,957 procedures to date and includes four operating rooms and a designated suite for gastrointestinal endoscopic procedures. The ASC provides services in a wide array of specialties, including ENT, orthopedics, urology, gastroenterology, podiatry, pain management and general surgery.

Neurological Institute of Savannah (Ga.) ASC. The surgery center offers a bundled pricing option for all spine procedures, including cervical and lumbar decompressions, discectomies, fusions and arthroplasties. The bundled price includes the surgeon fees, anesthesia fees, facility fees and a 30-day professional global period. Currently, the bundle option is available for only cash-pay paients, but negotiations are underway for use in hospitals without neurosurgery services and companies with self-funded insurance plans.

The ASC is accredited by the Accreditation Association for Ambulatory Health Care and includes 10 credentialed physicians. It is a part of the Neurological Institute of Savannah. The practice offers surgical services in numerous specialties, such as spine, trauma, pain management, vascular surgery and pediatrics.

Orthopedic Surgery Center of Orange County (Newport Beach, Calif.). The surgery center has two bundled payment models — one for cash-pay accounts and one for a third-party administrator that coordinates medical tourism cases for employers. Initially, the programs began with rates covering care given from admission to discharge. It then evolved into a bundle including a 90-day warranty for joint replacement at the affiliated hospital, Hoag Orthopedic Institute, according to a Becker's ASC Review article.

"The process is one of extreme simplicity when managing bundled cash patients to one of more complexity when working with a bundled arrangement that offers a warranty," said Gabrielle White, RN, CASC, executive director of ambulatory services and network development with Hoag Orthopedic Institute and OSCOC.

The global cash/self-pay rate includes fees for the facility, surgeon, surgeon assistant, anesthesiologist, radiology, implants and overnight stay for certain procedures.

Established in 1999, the 14,000-sqaure-foot surgery center is a subsidiary of Hoag Orthopedic Institute. It is accredited by the Accreditation Association for Ambulatory Health Care and includes four operating rooms. The center provides a wide array of services, including fracture repair, hand surgeries, spine surgeries and total hip replacement.
