1. Nashville, Tenn.-based Envision Healthcare (AmSurg): 261
2. Addison, Texas-based United Surgical Partners International: 255
3. Deerfield, Ill.-based Surgical Care Affiliates: 219
4. Nashville-based Surgery Partners: 125
5. Nashville-based HCA Healthcare: 123
