Rhode Island Officials Investigating 5 Hospitals for Alleged Medication Mix-Ups

State officials are investigating five Lifespan hospitals after 2,000 patients were given the wrong type of their medications, according to a Boston Globe report.

The five hospitals in question are Newport (R.I.) Hospital, Bradley Hospital in Riverside, and Providence's Miriam Hospital, Rhode Island Hospital and Hasbro Children's Hospital. According to the report, 2,000 patients from these hospitals were given prescriptions for timed-release medications but may have had prescriptions filled for the regular form instead.

Lifespan said it has notified 90 percent of affected patients, many of whom have already corrected their prescriptions. The hospital operator attributed the error to a software malfunction. So far, no patients have been harmed by the error.
