The Agency for Healthcare Research and Quality offers a free staff safety assessment through its program "On the CUSP: Stop BSI," which uses the Comprehensive Unit-based Safety Program to prevent central line-associated blood stream infections.
The assessment is designed to help staff identify patient safety risks in healthcare units and develop solutions that will eliminate these risks. The form asks providers how a patient in the unit could be harmed and what they can do to prevent or minimize this harm.
