The Patient Engagement Framework includes five phases that change a provider-centric model of care to a patient-centered model: inform me, engage me, empower me, partner with me and support my community. Each phase is designed to help healthcare organizations build capabilities to engage patients, including those needed to meet certain stage 1 and 2 meaningful use requirements.
