The Commonwealth Fund published an action guide for healthcare leaders on reducing avoidable hospital readmissions.

The leadership action guide includes an outline of four steps to reduce readmissions:

1.    Examine your hospital's current rate of readmissions.
2.    Assess and prioritize your improvement opportunities.
3.    Develop an action plan of strategies to implement.
4.    Monitor your hospital's progress.

The guide also includes specific strategies during hospitalization, discharge and post discharge to lower readmissions.
