The patient safety rounds help providers identify and eliminate patient safety risks through two methods: Staff communicate with each other about actual and potential safety problems, and former patients and their family members interview current patients about their perception of safe care.
The toolkit includes organizational assessments, program guidelines, training materials and other resources.
