The Minnesota Hospital Association announced Minnesota hospitals participating in the Partnership for Patients Hospital Engagement Network have prevented more than 3,200 readmissions since 2009.
The Partnership for Patients Hospital Engagement Network includes 26 hospital engagement networks across the country working to reduce hospital-acquired conditions and reduce preventable hospital readmissions.
