Healthcare industry lagging in adopting patient-centered, coordinated care model: 7 things to know

Health information technology is enhancing care coordination and the quality of care, based on a Council of Accountable Physician Practices-sponsored survey, according to Medscape.

Nielsen Strategic Health Perspectives conducted the consumer survey involving 30,007 participants.

Here are seven things to know:

1. About 50 percent of patient respondents said they had received care from a coordinated team.

2. Thirty-nine percent of chronically ill patients received help from a care manager compared to 37 percent of all patients.

3. Of the patients, 60 percent noted their primary care physicians could access their hospital visit records.

4. About 50 percent of patients noted their physicians knew their history before their appointments.

5. Only one-third of patients reported they had 24/7 access to care outside of their hospitals' emergency rooms.

6. About 5 percent of patients reported their PCPs asked them to use a mobile app to measure their physical activity.

7. Researchers concluded the industry is moving in the right direction toward a patient-center, coordinated care model, but at a slow pace.
