Here's what you should know:
1. The insurer is collaborating with Bethesda, Md.-based Aledade, a healthcare provider established in 2014.
2. Blue Cross and Aledade are aiming to help independent physicians use technology and data analytics resources to coordinate care.
3. Primary care practices focused on internal medicine, family medicine or general practice for adults are eligible to join the ACO.
4. The ACO is designed to improve patients' access to primary care while reducing preventable hospital stays, emergency room visits and unnecessary services.
