A new article in The Joint Commission Journal on Quality and Patient Safety explains how The New York State Anticoagulation Coalition developed a list of requisite data elements to manage the use of anticoagulants of patients in a new care setting.

The article, titled “Defining Minimum Necessary Anticoagulation-Related Communication at Discharge: Consensus of the Care Transitions Task Force of the New York State Anticoagulation Coalition,” by Darren Triller, PharmD, president and CEO, WellScriptEd Consulting and co-authors, details 15 requisite data elements the task force identified for anticoagulation communication discharge.
2. The requisite data elements include:
• Anticoagulants the patient currently uses
• Indications for anticoagulation therapy
• Documentation showing if the patient had previous anticoagulation therapy, or is new to the therapy

3. “Effective solutions to improve anticoagulation safety and effectiveness will be complex, but the ACDC List is an important contribution to improving anticoagulation management during one of the most vulnerable periods of patient care,” said Nadine Shehab, PharmD, senior scientist, Medication Safety Program, Division of Healthcare Quality Promotion, Centers for Disease Control and Prevention in an accompanying editorial.
