CMS has mandated accredited organizations report certain infection control breaches to the state, beginning June 30, 2014.
When any of the following breaches are identified during a Medicare Deemed Status Survey, the accrediting organization or state agency will alert state public health authorities:

When any of the following breaches are identified during a Medicare Deemed Status Survey, the accrediting organization or state agency will alert state public health authorities:
- Using the same needle or injection device for more than one individual.
- Re-using an injection device that has already been used to administer medication to an individual to renter a medication container and use the contents on another individual.
- Using the same lancing/fingerstick device for more than one individual, even if the lancet is changed.
